Referrals are taken 24 hours a day. After hours, referrals will be processed by our on-call staff. The following information is helpful at the time of the referral:
- Patient name, address & phone number
- Diagnosis, social security number, & date of birth
- Attending physician's name & phone number
- Insurance information
- Caregiver or family contact name & phone number
A designated Infusion Primary Team member will initiate and coordinate all home infusion needs in conjunction with the facility discharge planner, social worker, or other healthcare professional.
Comprehensive patient education and training begins at the pre-discharge visit by a Care Coordinator and will continue at the patient's and/or caregiver's own pace of learning. Our own licensed Pharmacy dispenses all medications to ensure prompt, accurate and on-time deliveries.
Throughout the home infusion therapy regimen our staff maintains regular communication with the patient and performs additional follow-up visits according to individual patient needs. We coordinate any additional health services needed including reimbursement services, and function as a central liaison between healthcare professionals through the course of home infusion therapy. Our team is available 24 hours a day, 7 days a week.
