PARTners Program (Palliative Care in Alternative Residences Team)

This is a distinctive service provided to residents of long-term care facilities and assisted living facilities in our area. Hospice and facility staff work together to ensure the residents receive optimal end of life care.

Hospice care is available to everyone, no matter where their "home" may be.
